A handful of times at a few hotels.
It generally is not on purpose..it's when the hotel has a newish LCD set, and the cable system feeding the hotel has locals digitally on QAM mixed in with the analog lineup.
Using our TWC system lineup as an example:

You get the idea. The set scans in the QAMs, and because the cable company actually provides PSIP info, the digital/HD versions land in the middle of the analog lineup...thus, analog 4 is WOIO, digital 19.1 is WOIO's HD feed, and digital 19.2 is Me-TV.
I've seen this in about three or four hotels that have put in LCD sets and just hit "scan".
If the cable company doesn't carry a sub, it won't show up. If the hotel uses a satellite based system, the subs won't show up, as satellite generally does not carry subchannels.
And if the hotel has its "own system", this won't happen, either.
