I'm not sure of the exact date of this lineup, but it seems to be from either the late 80s or early 90s, since a lot of the cable channels are newer. I'm surprised they were still using one of these cable boxes this late.

From the early 1990s. WBAY and WFRV swapped affiliations on March 15, 1992 WBAY to ABC, WFRV to CBS.

It had to be 1992 or later, as WGBA didn't pick up the FOX affiliation until 1992 (when then-WXGZ went silent).

I’m also interested in why they would be using this type of cable box this late, and it makes me wonder how many cable companies still were. I remember going to a hotel in Maine in 1998 where they were still using them. But that’s because things don’t change very quickly in Maine. Maybe Green Bay is the same way.
 
This is an interesting lineup. Pay-TV Disney on 6 (in lifeline service?) I suppose it was scrambled to non-subs and that's where they could put it. Also TWC on 11 but WGN is NOT in lifeline, despite being less than 200 miles south.

Also no WVTV during this era! WVTV was re-added in the mid to late 1990s, likely when it picked up The WB (Green Bay didn't have a full-time WB affiliate until 1999)
