Again, there is no such rule. As an example, Wilks Broadcasting owns both KFKF and KBEQ in Kansas City, and both air country with very little difference in playlists.

They cannot air original programming on a translator it has to be either HD2, 3, or 4 or as a repeater of a FM or AM station.

Correct, but with one exception. If the primary station for an FM translator is a daytime-only AM, it can originate after hours programming on the translator.

Cox could (but won't do HD2 in San Antonio) on KONO HD-2 as Classic Country, repeat it on 104.9 and simulcast KONO 860 AM on 680 AM.

No, they couldn't. AM-AM and FM-FM simulcasts are illegal if the stations have similar coverage areas. Without a waiver, simulcasting 860 on 680, or vice-versa, isn't permitted. Repeating an HD2 on a translator, however, is legal.
