As of January 13, 2012, the Nationwide EAS Test Reporting System is no longer accepting electronic filings. If you have not filed a mandatory report as of the deadline of December 27, 2011, either electronically or via paper filing, we will continue to accept a late filed paper report.
To file a paper report, you must file an original and one copy of each filing with the Commission by hand, messenger delivery, commercial overnight courier, or by first class or overnight U.S. Postal Service mail addressed to the Secretary, Federal Communications Commission, ATTN: Nationwide EAS Test, 445 12th Street, SW, Washington DC 20554. EAS Participants must mark the top of each page of their submissions: “Confidential – Not for Public Inspection.” [U.S. Postal Service first-class, Express, and Priority mail must be addressed to 445 12th Street, SW, Washington DC 20554. Commercial overnight mail (other than U.S. Postal Service Express Mail and Priority Mail) must be sent to 9300 East Hampton Drive, Capitol Heights, MD 20743. All hand-delivered or messenger-delivered paper filings for the Commission’s Secretary must be delivered to FCC Headquarters at 445 12th St., SW, Room TWA325, Washington, DC 20554. The filing hours are 8 a.m. to 7 p.m.]
Instructions For Paper Filings
The Nationwide Emergency Alert System Test Reporting System is designed to collect information on the results of the National Test on November 9, 2011. Broadcasters and Cable Operators are asked to provide information on the success or failure of the test.
Paper submissions must include the following information for each facility:
Participant Information
Legal Name of EAS Participant
FCC Registration Number
Type of EAS Participant: Broadcaster or Cable Operator
If Broadcaster, the Facility ID number and Call Sign of the station
If a Cable Operator, the CUID and PSID for the headend
Transmitter/Headend Location
Provide the latitude and longitude of the facility
System Specifics
EAS Designation: State Primary, State Relay, Local Primary 1 (LP-1), Local Primary 2 (LP-2), Participating National (PN), Non-Participating National (NN)
Station you monitor for EAS identified by Call sign
Any alternate stations you monitor identified by Call Sign
Your EAS Equipment Manufacturer and Model Number/Software Version
System Performance
Did you receive the emergency alert? - Yes or No
If you received the alert, were you able to pass it on to all communities? - Yes or No
Explanation of success or failure of receipt of the EAS, including any technical information on equipment performance
Date and Times
Provide the message receipt date
Provide the message receipt time
Provide the local time zone
