Ford-Carter Debate Audio Failure (1976)

Regarding the famous audio glitch that interrupted the first of the Ford-Carter debates in 1976.....what was ultimately determined to be the cause of the outage? And did the incident cause any changes in coverage of such events, such as insuring that a backup audio feed is available, etc.?

One of those funny moments that tend to crop up during such unexpected glitches......the great David Brinkley, filling air time while waiting for the problem to be fixed, uttered one of the most "Captain Obvious," tautological statements of all time:

"I can't hear them...so I don't know what it is we're not hearing."

Uh, yeah.....thanks for the heads up on that, David.... ;D
