Hip-hop isn't big in Spain, so no stations. Don't know Portugal at all. In Italy you get Play Radio which does some urban stuff these days, and has relays in most cities and large towns. There may be a local hip-hop station somewhere - there are so many stations in Italy it's impossible to be 100% sure...
France has a LOT of hip-hop stations. Skyrock is national - it's the world's biggest single urban format station, with hundreds of transmitters nationwide.
The other Parisian stations are Ado FM 97.8 (more R'n'B style) and Générations FM 88.2 which plays a lot of local hip-hop. There are also a few community stations in the Parisian suburbs like Vallée FM 98.4, IDFM 98.0 which play a lot of hip-hop. Many other French cities have local stations playing a lot of hip-hop. There's Vibration 89.9 in Brussels, over the border in Belgium, where Skyrock doesn't reach.
Up in Scandinavia (Denmark / Sweden / Norway) there's The Voice, which is available in most cities.
Here in the UK we have Choice FM, which only covers about 50% of London (and plays very little UK hip-hop), although there is a digital-only station from the BBC, 1Xtra, which has a lot of urban stuff. There are also shows on community stations and on stacks of pirate stations.
Hip-hop isn't the big underground music in the UK though, unlike in France. We also get garage/grime/dubstep, which cover a kind of dance/urban crossover. They can be heard on loads of pirates in London. Very few of the pirates have working web streams though. I know you can hear dubstep and garage, and occasionally grime, on Rinse FM, which definately has a webstream - other than that I'm not sure...
