On The Big Major Stations I'd imagine not often, if ever.

Sounds like someone forgot to pot down the Phone on the board.

Actually, I called in to the station, while that tone was being broadcast, and the person that answered, said that "there was nothing that they could do about the Hold Tone" being broadcast....

Not being an engineer, I could not be of any further help to the station.

But, it is aggravating to hear that tone sound every 30 seconds ! ! :-\
 
Maybe the telephone hybrid needed rebooted? From what you described it sounds like a line got "latched" and would not release. If that was the case a reboot is the fix..
