Hotels with actual HD programming

Last night I went to Loma Linda hospital in Murietta CA (Family emergency) and the big screen TV was showing ESPN in SD strechovision. When I asked to switch to the super bowl she grabbed the remote and channeled down. I noticed as the channels switched there were ##-1 channels and told her to stop on 5 & to click once more for 4-1 where she got KNBC-HD. My guess is either the cable had ESPN in the clear or the TV (Visio) had a cable card.
If the hotel/motel uses local cable I'm sure it's plugged directly into the TV and you'll get local HD and maybe premium HD if there is a cable card in the set.
The only time I've seen HD was when I stayed at a hotel in Nashville in August of 2010. They had a 32" Sanyo LCD HDTV, which was only different in screen size from the Sanyo I happen to own from 2006 (which still function fine). While the analog reception was crappy or so-so, the QAM tuner on this TV was awesome! Same remote and all, so, needless to say, a digital scan was done immediately. All the big Nashville locals came in with HD glory. The hotel's TV didn't have any kind of converter attached to it.
I don't think Hotels are actually paying for the true HD feed though. It may just be there by luck.
When do we get to the point that we no longer pay and extra fee for HD? Considering it's pretty much standard now and over 2/3rds of the outlets are broadcasting in HD. It would be like 50 years ago having to pay extra for color.
