How does a format change originate?

I've been on these boards for over a decade, but I don't think I've ever seen this question asked.
What I'd love to hear from Those In The Know and Those Who've Been There Done That is how does it all start? What was the genesis of a format change? Was it by one person saying HEY I think this market/station needs a ****** format!! Or was it a group of people, a board meeting, a corporate meeting, hashing it out and throwing out ideas. Any and all thoughts will be sincerely appreciated! :)
 
In the ones I have been involved in it was an ownership change, or just a decision by an owner or GM. Other times a consultant arranged a meeting with an owner or GM and would convince them to change. That's how it was back in the "old" days. More "modern" methods invove a research study being employed to determine a new format for a failing station.
