KGTV News Bumbling

And this must be from a couple years back because Lisa Lake hasn't been at KGTV for at least that long.

This got national attention, it ran on Late Night with Jimmy Fallon last night.

Having watched the video and having worked in live TV news prodcution, here's what went wrong, from what I can tell:
1. Cold Open reporter wasn't ready or wasn't told to "standby"
2. Cameral 2 2-shot wasn't ready
3. Anchors were unprepared to go live and should have also been on "standby"
4. Live shot wasn't ready, wasn't on standby, and wasn't told to "cue up" the reporter

Also, were these people given countdowns?
A countdown should have been given before numbers 1 and 4.

Perhaps they were training a new production director, new crew or both. Or, they just had a bad morning.
 
Channel 10 is totally automated in the studio with robotic cameras. A producer handles all of the camera movement with a joystick and handles the teleprompter at the same time.

Channel 10 News does not do cold opens with a reporter. Rather it looks like a severe switching mistake combined with all of the other errors.

What is interesting is that the video went viral, disappeared, and has resurfaced even bigger.
