LPB S-12 Console

In one of my production studios, I have an LPB S-12 console. It has a low hum that is only audible when there is no audio running through it, or the audio level is low. I thought it was electrical interference from the AC power lines in close proximity with the audio lines, but I separated them completely and it did not go away. I also pulled out the program cards and the hum went away. Any ideas where this hum is coming from? Also, I need a replacement cue card for it. Does anybody know where I can find one?
 
Make sure the screws that secure the main circuit board to the frame of the LPB are tight. Also, if it has the single vertical power supply cap, make certain the screws in the bottom of the cap (the ones that make contact with the circuit board) are tight.
