Monitor mute add-on box or circuit

Broadcast Tools used to make two different switch boxes to use with non-broadcast (e.g. Mackie) consoles that muted the monitor when you switched the mic on. For some reason they have discontinued both the CC-5 and CC-2. Does anyone make a comparable box? I need two.

Failing that, anyone have a good circuit/schematic for one?

Actually, it is not that hard to build one. Years ago, I found an old Western Electric 4PDT lever-switch in a box. I think the phone company used them for switching between two lines, or turning off bells, signal lights, etc. In any case, you could find a modern multi-pole switch and put it in a box. All you need to do is use one pair of contacts to interrupt the "insert" jack on you mixer. The other poles can be used to mute your monitor feed and even turn on an on air light (via an external relay).
