Monitoring suggestion?

I have a need for a very basic box that will accept around a dozen balanced input feeds from various sources, selectable by a rotary switch perhaps, has a stepped attenuator (front panel adjustable) and some kind of level display be it LED VU or meters.
It's to go in a control room rack to listen to various sources.

I need something off the shelf.

Any suggestions?
 
If you can use external metering then I would probably build my own. I've used a rotary switch from an audio console (one for the remote lines inputs) mounted it in a project box or on a rack pannel and used a barrier strip for the connections. Cheap and easy. I know its not "off the shelf" but its close.
