NBA games blacked out or on Pay Per View?

In the "NBA on TNT Overrun" thread, there is discussion about how San Antonio Spurs home games (on TNT, ESPN, or Fox Sports SW) are blacked out if they aren't sold out. Are there any other teams that do this?

And are there any teams that still have games on Pay Per View? As I mentioned in that thread, the Phoenix Suns began this practice in 1993 (1993-94 season - the year after Charles Barkley led them to the NBA Finals) and ended it very shortly before the start of the 1998 playoffs. I'm guessing no, considering what I assume is a general decline in the usage of PPV - but I'm curious.

In the Suns' case I bet another declining factor was the fact that there are so many small towns and small cable systems out here. By keeping the games from TV some small town fans probably missed out.
 
I don't think any NBA games are allowed to be on PPV because the NBA sells its own NBA League Pass package for about $200/year where you get to see pretty much every game played. If specific games were on PPV people would only pay a few bucks to only watch the good matchups.
