The Packers consider WTMJ 620 Milwaukee their "flagship," but in Green Bay itself, the games are on an AM/FM simulcast. And since no games are played in Milwaukee anymore...
According to this article, the teams with AM-only flagships are Buffalo, Denver, Carolina & the NY Giants. The others are either simulcasts or FM only.

IIRC, the Giants games were moved to FM when there were Mets conflicts this past season. I'm guessing that they'll have their games on FM fulltime in 2013, either on WCBS-FM or WFAN-FM, as well as AM 660 when there's no Mets game.
In any case, the NFL on AM may not be completely dead yet, but it's heading that way, at least for local broadcasts, and has been for about 20 years.
