We stayed in Page in the early 2000s and I do not recall KNAZ being on the cable there. AFAIK the Utah/Arizona state line is an absolute dividing line between SLC and Phoenix market stations, with no overlap aside from PBS (as in Page).

As a side note, Coconino County is so huge, that we stayed one night in Page, went on to the Grand Canyon, then stayed in Flagstaff for a couple of days, not leaving Coconino at all, aside from a day trip to Sedona where we grazed Yavapai County. Coconino is larger than 9 of the 50 states.

Many market boundaries are absolute with no overlap, especially in the western states where the markets cover area hundreds of miles from the primary cities.

I realize that, I was just referring more to the fact that you will not find Phoenix stations, not a single one, on cable in southern Utah, nor SLC stations in northern Arizona, even though they are just as accessible on one side of the border as the other, e.g., SLC stations are able to get to Kanab, near the border. As for the intricate translator and microwave networks in Utah, which deliver SLC TV to every corner of the state, it wouldn't take that much more, for instance, for Page to get one or two SLC stations, but they don't, aside from PBS. I don't know if that statewide network is used for anything except OTA reception anymore, as I'd assume that SLC stations can use satellite transmission to feed their translators --- but do they?

Dumb question, how common is it for cable systems (or translators, for that matter) to take feeds directly from Dish Network or DirecTV? And would the signal, as received, be of sufficient technical standards to be usable for cable or translator distribution? I heard of one case where a Roanoke station (I want to say it was the CW affiliate) got into Beckley WV cable that way.

Things were a little blurrier 40 years ago, though market boundaries have hardened due to syndex, incorporation of ADIs/DMAs into certain FCC regulations, and other factors.

If there had been a cable system in Colorado City at this time, it would have been almost all Salt Lake. That area currently receives almost all network service from Salt Lake but Fox from KSAZ in Phoenix.

Yes, the microwave links are used as part of the transmission chain. The air chain to St. George consists of over-the-air reception at Levan -> Frisco Peak -> Beaver Dam / Utah Hill / West Mtn. -> St. George, with mostly microwave provision down the line from Levan. At each hop except Levan, there are dependent translator sites.
 
Speaking of KSAZ, they have a translator (owned by Mohave County) that is licensed to Littlefield but broadcasts from the main St. George tower farm, giving it a signal that covers Mesquite (NV) as well. If Littlefield/Beaver Dam had cable, it is likely that it would've carried mostly Las Vegas and SLC stations with few or none from PHX.

Laughlin and Bullhead City, which are on opposite sides of the Colorado River, are served by the same cable company, but have completely different locals (Laughlin is in Clark County NV and gets only Las Vegas stations, while Bullhead City AZ is in Mohave County AZ and gets only Phoenix stations; however, many stations from both markets are available OTA via translators)
