Back in '97, Pioneer created DVD-R. By decreasing the thickness of the polycarbonate layer, the beam's spot size can be smaller for creating more pits...therefore, more storage. DVD-R tracks by having land pre-pits between the grooves and that controls the wobble frequency.
DVD+R was created by Sony and Philips in 2002 because they're jealous. The difference is that DVD+R tracks by measuring the distance to the outside of the disc to determine the wobble frequency.
Determining the exact cause for problems is difficult as it relies on each machine's unique mechanical manufacturing and it's firmware. When the motor control or beam alignment are off, the disc skips...it's usually from dirt on the media, vibration or loss of tracking (lost wobble frequency).
