People frequently mentioned but never seen

Steve Urkel's Parents
Kimmy Gibler's Famiy on Full House
The parents and relatives on Beavis and Butt-Head(expect for their dads in the movie)
 
Adults in the Peanuts cartoons. They were never seen, and if they were supposed to talk, it was via a "wah-wah" trombone.
 
Sarah the telephone operator on The Andy Griffith Show
Except for the pilot for Emergency! where she made an appearance, Roy DeSoto's wife JoAnne or their kids were never seen but were mentioned quite a number of times by Roy either by talking about her or talking to her on the telephone.
Jim Reed's son on Adam-12. Reed's wife was seen in the first two seasons and then in the last season played by Kristen Nelson. Their son should have made an appearance since by the last season he would have been 5 or 6.
 
Maris, Niles first wife on Frasier.
Vera, Norm's wife on Cheers, though a pie covered person showed up on a Thanksgiving episode. Her face was never seen.
 
Larry Mondello's sister on "Leave It to Beaver." She must have been a mess.....

And the granddaddy of them all, John Beresford Tipton
of "The Millionaire"; we usually saw the back of his head
as he sat in a chair, and an arm and hand when he gave
Michael Anthony the check to deliver. I think he might
have been seen once, when Anthony was arrested while
delivering a check and Tipton had to come down and bail
him out. (But there was no mistaking the voice of Paul Frees.)
 
...although it's a TV-movie rather than a series, one of the most famous characters of this type is the trucker who stalks Dennis Weaver in Duel. All we see of him is a left arm waving Weaver ahead and his boots as he kicks the tires on his truck at the gas station...

Larry had a brother too, he used to punch Larry in the arm, "Right on the arm," as Larry lamented. That is until he got married and moved away

And, nobody mentioned Gladys, Pete's wife (was it "Burns & Allen?")---of course, Gladys finally materialized on the sitcom "Pete & Gladys."

By "never seen" - can we cheat a little and include characters who were heard - but never seen (in full, anyway, on camera). of so, I'll mention George Steinbrenner, who was heard (voice of Larry David), but only seen from the back.

In the same category, I'll mention John Beresford Tipton (The Millionaire).

In the never-seen-or heard category - Pete's (Harry Morgan) wife Gladys, who he always complained about on December Bride. Of course, Gladys was finally seen in the spin-off series, Pete & Gladys.
 
Well we'd have to include Wilson from "Home Improvement," but I'm sure if you piece together all the images from all the episodes, you could get the full face!

So, it was "December Bride". I have seen both "Bride" & "Burns", but not much, so I sit corrected.

Wilson was seen in the final episode, or maybe the outtakes or credits, if memory serves.
 
On The Drew Carey Show, Drew's first boss, Mr. Bell, was never seen; only heard through Drew's speaker phone. (Although he was finally seen in the episode in which he was fired; he was shown carrying a box of his belongings out of his office.) His replacement, Nigel Wick (played by Craig Ferguson), was, of course, seen all the time.

On WKRP in Cincinnati, overnight jock Moss Steiger was mentioned, but never seen. Midday jock Rex Erhardt may also qualify, although he was finally shown once, in an episode in which he filled in for Dr. Johnny Fever. (He was played by Sam Anderson, one of many one-time roles Anderson played on WKRP.)

If we can count cartoons, Dr. Claw from Inspector Gadget qualifies as well. Only his right hand is shown and his voice is heard; his face was never shown.
