Rick Williams "flub-up"

I was surprised tonight (Fri/12-8) to see Rick Williams make a pretty easy mistake--not once, but twice in the same "breaking news" segment. Chopper Six was live over a crash on West Chester Pike in Broomall and Rick was relaying the information--giving the details of the crash and mentioned how medics were "extraditing someone from the vehicle". As you know--"extradite means: to give up (an alleged fugitive or criminal) to another state, nation, or authority"...of course--he meant to say---"extricate". But then, not a few seconds later--he mentioned again---"as you can see from Chopper Six--Firemen and medics are extraditing someone from the SUV". Easy mistake--but TWICE? Certainly couldn't have been a slip of the tongue...wondering if someone typed it in wrong on the prompter?
