Sample Voice Over Contract

I am about ready to sign some imaging/commercial clients but have no sample contract to go by. I'm not interested in your rates, I've established my own, I just need a sample contract that spells out terms of payment and protects me.

Anyone willing to send one and help a fellow broadcaster?
