They didn't carry KNTV-11?

I know it was a Salinas/Monterey market station (this even though San Jose is physically outside that market), but seeing as it was in the Bay Area, you'd think they would.

Actually, KNTV's transmitter was about 60 miles from San Francisco, so even though San Jose proper is in the SF market, it wouldn't have been that different from its being a Sacramento-market station. KNTV was in the odd position of being licensed outside its own market. Evidently it wasn't deemed worth it to carry it on cable in SF. They could always have sim-subbed it, laying KGO's network programming over top of the KNTV schedule.

It's a similar situation to WMUR in Manchester NH or WWSB in Sarasota FL. Viewers in Boston or Tampa might be able to get them with a good antenna, but as a practical matter, they probably wouldn't bother unless there were an attractive syndicated offering (such as the evening game shows out of pattern with whatever Boston or Tampa station also carried the show) or unless WCVB or WFTS pre-empted an ABC offering. ABC seems to have more of a tolerance for dual affiliates at far edges of the same market, at one point NBC ditched several small-town affiliates (Philadelphia, Washington DC, Salt Lake City, possibly others).

Here are KNTV's contours from the 1995 Television Factbook:
