To be a bit more concise...

ALL possible combinations of KTV-, WTV-, K-TV, and W-TV are in use with the following exceptions:

KTVY
WTVN (on AM 610 Columbus, Ohio but not on any TV station, full- or low-power)
WATV (on AM 900 Birmingham, Alabama but not on any TV station)
WSTV (on AM 1340 Steubenville, Ohio and FM 103.7 Frankfort, Kentucky but not on any TV station)

Actually, there are probably hundreds more, as the -TV suffix on those stations that have it is legally part of the calls.

There are a CFTV, (Leamington, Ontario) CITV, (Edmonton) and CKTV (Saguenay, Quebec) in Canada. And again, actually, *all* private TV stations in Canada have -TV as part of their callsigns. (as do some CBC stations)
 
Actually the old analog ch 9 in Steubenville OH once had the calls WSTV-TV, but apparently the STV stood for Steubenville, and "TV" was not emphasized, hence WSTV-TV.

And by the same token there is WATV AM 900 in Birmingham....no relation to "TV" at all.

Here are the stations in the Chicago market with the -TV suffix:

WBBM-TV (because they also have -AM & -FM on the radio side)

WMAQ-TV (they used to have 670 WMAQ, but was sold to Group W, the predecessor to the current CBS)

WLS-TV (they [ABC] used to own WLS-AM 890 & WLS-FM 94.7, & those calls are still on the radio side, but now owned by Citadel)

WGN-TV (They own WGN-AM 720)

WCIU-TV (not sure why, but they've ID'd with -TV for many years, & been on the air since 1964)

WWTO-TV (not sure, but this station only covers the western suburbs of Chicagoland, & many still can't get them, even after going to 80kw on RF 10)

WCPX-TV (not sure, but they've ID'd that way since taking over the former WCFC in 1998)

WSNS-TV (Never knew they Id'd with-TV)

WPWR-TV (not sure, but they've Id'd that way even while on channel 60 back in the early 1980's)
 
And then we could start dipping back into history to find stations like the original WXTV, channel 45 in New Castle PA/Youngstown OH, or WATV 13 Newark NJ (later WNTA-TV/WNDT and now WNET), or WTVU Channel 73 (!) in Scranton, or WTVH 19 in Peoria IL (now WHOI), or KBTV 9 in Denver (now KUSA), or KTVY 4 in Oklahoma City (previously WKY-TV, now KFOR)...but that way lies madness...
 
Surprisingly, not mentioned til now is longtime ABC/Fox/CW affiliate, XETV, channel six (still broadcasting in analog, Mexico hasn't made the change just yet), Tijuana/San Diego.
 
The blanks all once had TV stations except for WATV, but I'm only mentioning some not featured:

KTVV — 36 Austin, TX. Changed 1987 to KXAN.
KTVY — 4 Oklahoma City. Changed 1990 to KFOR-TV.
WTVN — 6 Columbus, OH. Changed 1987 to WSYX.
