Studiohub headphone panels

We have had a few Studiohub headphone panels that have had a channel go dead, anyone else have that happen? We supply power to one, then gang it through to the next 3, 2 of which had one channel die out.

Contacted Studiohub a week ago and never heard back so hoping maybe someone out there can help us save a few bucks :~)
