"Talk" Button Errors on Fan Drive Me Crazy

Heaven help me, I end up listening to the fan most of the day in my office. Every hour on every show, it seems, there is at least one instance of a host being muted because they haven't pushed the "talk" button. It's across the board with every host it seems.

I am not in the business. Is this an incredibly complicated bit of technology? Is there any reason why staff who have been on the air at this station for well over a year still have problems with this? Why do they tun the mics off any way?
