Exactly how are the total number of people watching measured?

The way it's always measured:

Philadelphia’s big win in the bayou peaked at 137.7 million viewers in the second quarter (8:00 PM – 8:15 PM ET), per Nielsen Media Research.

 
“TV Off???” Kendrick Lamar kept it “Humble” with 1.335 million viewers tuned on!
and most of those viewers who opted to keep it on during halftime, wanted to see if he would sing the song where he badmouthed Drake.

Or they just wanted to hear and see the show. He is a far more multitalented performer known for so much more than that. He’s not my thing, but he is very much a star and excellent at what he does.
 
The way it's always measured:
Great response. Let me be more specific...

How are the number of eyeballs counted? For instance, if 9 people are watching the game how does the tabulating company know there are 9 people vs just counting the one TV set in operation and estimating an average? Or do they take just a sample then pro rate? If the big screen is out on the patio but all the people are drinking beer and playing cornhole how do they count those people?
